Please note: You must register a trip with the Collegiate Challenge team before submitting these forms. You will need Adobe Acrobat Reader to view or print these forms.
Confirmation form
This form must be submitted to the Collegiate Challenge office within 10 business days of registration along with the $20 per person program fee, or the trip will be canceled. We use this form to officially register your group for the date, site and number of participants you have registered. Please submit one form for each trip. Once you have submitted this form, you are responsible for all costs associated with this trip. Please visit What You Need to Know to view the cancellation policy.
Team roster
Our team roster form, necessary for insurance validation, is now online. Team leaders should complete a separate team roster for each team your group is sending. All team rosters must be complete at least two weeks prior to your trip. Completing the team roster will qualify your team for a 24-hour supplemental medical insurance policy that will cover each listed team member during the trip. Please visit the What You Need to Know section of this site for more information on the insurance policy.
Release and waiver of liability form
Each team member must complete this form. Team leaders need to collect and return these forms to the Collegiate Challenge office at least one month before the start date of the trip.
Emergency contact form
Each team member must complete this form in case of an accident or medical emergency. These forms should be kept together with the group leader throughout the trip.
