Please note: You must register a trip with the Collegiate Challenge team before sending in these forms.

Confirmation form

This form must be sent in to the Collegiate Challenge office within 10 business days of your registration along with the $15 per person program fee, or your trip will be canceled. We use this form to officially register your group for the date, site and number of participants you have registered. Once you have submitted this form, you are responsible for all costs associated with this trip. Please visit the What You Need to Know section of this site to view the cancellation policy. Please submit one form for each trip.

Insurance validation form

Group leaders must return this form to the Collegiate Challenge office at least one month before the start date of your trip. This form will qualify your team for a 24-hour supplemental medical insurance policy that will cover you during your trip. Please visit the What You Need to Know section of this site more information on the Insurance Policy.

Release and waiver of liability form

Each member of your team must fill out this form. Group leaders need to collect and return these forms to the Collegiate Challenge office at least one month before the start date of your trip.

Emergency contact form

Each member of your team must fill out this form in case of an accident or medical emergency. These forms should be kept together with the group leader throughout the trip.
