Program resources for Collegiate Challenge

Please register for your trip with the Collegiate Challenge team before submitting these forms. You will need Adobe Acrobat Reader to view or print these forms.

Participant waiver forms

Each trip participant must complete our participant waiver form.

For participants under the age of 18, team leaders should collect and return these forms by mail or fax at least one month before your trip start date.

Participants over the age of 18 can now sign our waiver electronically.

This team leader guide will assist you in the process of organizing your trip and leading your team.

Participant guide

Our participant guide contains helpful hints on fundraising, site logistics and reflections. We hope it will be a valuable tool that you will continue to use, even after you have returned from your trip.

Managing trip expectations

Helpful tips for setting clear expectations for participants before your trip.

Learn about raising concerns and how to confidentially and anonymously report any issue or concern that may arise during a Collegiate Challenge trip.

Emergency management

Each participant should complete the emergency contact form and give it to the team leader. These forms do not need to be sent to the Collegiate Challenge office. The team leader should keep these forms during the trip, in case of an accident or emergency. Team leaders should read the emergency management instructions to prepare for and learn how to manage medical emergencies during your Collegiate Challenge trip.

Trip information template for parent/guardian

This optional trip information form can be completed and provided to parents and guardians for Collegiate Challenge participants as a resource during the trip.
