Team roster
This team roster form is necessary for insurance validation.

  • Team leaders should complete a separate team roster for each team you are sending.
  • All team rosters must be complete at least two weeks prior to your trip. Completing the team roster will qualify your team for a 24-hour supplemental medical insurance policy that will cover each listed team member during the trip. Visit the frequently asked questions for more information on the insurance policy.

Participant waiver form
Each trip participant must complete this form.
For participants under the age of 18, team leaders should collect and return these forms by mail or fax at least one month before the trip start date.

Participants over the age of 18 can now sign the waiver electronically.

Emergency contact form
Each participant should complete this emergency contact form and give it to the team leader. These forms do NOT need to be sent to the Collegiate Challenge office. The team leader should keep these forms during the trip, in case of an accident or emergency.
